IMM 0008: Application for Permanent Residence in Canada.
This form must be completed by the
principal applicant.

 

Schedule 1: Background / Declaration
This form must be completed by the principal applicant and:

his or her spouse or common-law partner (if applicable);

and

all dependant children aged 18 years or older listed in the application for permanent residence.

 

Schedule 5: Economic Classes - Québec Nominee
This form must be completed by the principal applicant.

 

IMM 5406: Additional Family Information
This form must be completed by the principal applicant and:

his or her spouse or common-law partner (if applicable);

and

all dependant children aged 18 years or older whether accompanying the main applicant or not.

 

Immigration Summary and Supplementary Information Form
This form must be completed by the principal applicant and all applicants aged 18 years or older.

 

IMM 5476: Use of a Representative / Authority to Release Information to Designated Individuals
The principal applicant may complete this form.
 
Use this form if you want us to release your application information to another person or agency. Canadian law does not allow Citizenship and Immigration Canada to discuss your application with anyone without your permission.

IMM 5561: Instructions – Use of a Representative

 

Application for Police Certificate
The principal applicant and all persons in your family from 18 years of age require a police certificate / clearance from each country where they have lived for six months or longer since the age of 18.
