TRV Document Checklist For a Canadian Temporary Resident Visa The following documents are required to complete your application. If you are missing documents on this list, your application may be refused. All documents in a language other than French or English must be officially translated. Provide both a photocopy of the document and the translation in English or French. 1. COMPLETED application form. Please complete the fillable application IMM5257 and print it for submission to our office. If completed by hand you must print clearly so that the information can be easily read. If more than three (3) dependents are travelling to Canada, please complete a second application form to include the additional dependents. 2. COMPLETED supplementary information sheet (Part I & II). 3. Resumé/C.V. (curriculum Vitae) and additional questionnaires for nationals of any of the following countries (click on the appropriate one): · Afghanistan · Haiti · Iran, Iraq, Palestine Authority, Lebanon, Syria · Rwanda · Somalia · Sudan · Bosnia – Herzegovina, Croatia, Macedonia, Montenegro, Serbia (including Kosovo). For ease and speed of processing, we ask that you print your answers clearly using CAPITAL letters in each box. If a question does not apply to you, write N/A meaning “not applicable” in the space provided. 4. Original passport valid for at least 6 months and with at least one blank page. 5. Processing fee: cash, bank draft or money order. We DO NOT accept payment in CASH when an application is submitted by mail. We accept payments by Bank Drafts or Money Orders in the exact amount, for application submitted in person and/or by mail. 6. Two (2) identical passport sized photos taken within the last 6 months (name and date of birth written on the back). 7. For your common-law partner, statutory declaration of common-law union (IMM5409). 8. Proof of employment in your country of residence, current letter from employer and pay slips. If you are studying, please include a current letter and transcript from the educational institution you are attending. 9. Proof of funds (i.e. bank statements, proof of property ownership) 10. Documents relating to your trip such as an invitation from business of personal contacts in Canada, a letter from your employer, registration for a conference, your itinerary, etc. 11. Previously held passport(s) showing your travel history (if you have one). 12. For TRANSIT VISAS, photocopy of your airline ticket with confirmed flight bookings. 13. For minors travelling with only one (1) parent/no parents, a notarised consent letter from parent(s) not accompanying & birth certificate of the minor child. 14. A complete itinerary, cities you intend to visit, full names, addresses and telephone numbers of all personal and/or businesses you will contact, and your method of travel. 15. For diplomatic/official passport holders, note-verbale from your office stating the purpose of travel to Canada.
