WORK PERMIT FORMS

Application for a Work Permit.
These forms must be completed by the principal applicant.

Checklist.
The checklist includes documents which must be submitted by all applicants. It also lists additional documents required by applicants wishing to work in certain professions. This form must be completed and included with your application.

Live-in Caregiver Questionnaire.
This form must be completed by the principal applicant if applying as a live-in caregiver.

Low Skill Project Questionnaire.
This form must be completed by the principal applicant if applying under the Low Skill Program e.g. as a truck driver, food attendant, construction labourer or healthcare aide

Supplementary Information Form
This is required if you are a citizen of the following countries:

·        Afghanistan

·        Haiti

·        Iran, Iraq, Palestine Authority, Lebanon, Syria

·        Rwanda

·        Somalia

·        Sudan

Statutory Declaration of Common-law Union
This form must be completed by the principal applicant and common-law partner if applicable.
