Application form -
Each
applicant must submit a fully completed and signed application
FORM
(IMM1295B) which should include a day time telephone number. A
separate application form is required if your spouse wishes to apply for
an open work permit. Please refer to the Spousal
Work Permit page.
2. Processing
Fee -
Basic fee $150 per application
for work permit. See the FEES page for full information on the processing fees
which apply to you.
When posting
your application, DO NOT SEND CASH. IT WILL NOT BE ACCEPTED. The
fee must be paid by bank draft in Canadian Dollars and can be purchased at
most major banks. This draft should be made payable to:
RECEIVER GENERAL FOR CANADA
Please write your name and address and telephone number on the reverse of
your bank draft. You can also pay by postal order in British Pounds.
We can only accept a bankers
draft in Canadian Dollars or postal orders in UK pounds sterling for
applications made in person at our office for
Work Permits.
No cash will be accepted.
This fee is non-refundable
regardless of the outcome of your application.

Photographs -
Two passport sized photographs of yourself.
Please write your name on the back.

Detailed CV.

Medical Examination - When we receive your
application, we will issue your medical examination forms and
instructions. You must undergo a medical
examination with one of the designated medical practitioners on the list
who will send the results to us for review.
7.
Qualifications - Proof of professional qualifications/educational
certificates/apprenticeships - copies acceptable.
8.
Documentation - Proof that you are eligible for interim licensing/registration (i.e.
letter confirming eligibility) with the regulatory body.
Clinical Fellows destined to Ontario, Alberta or British Columbia do
NOT require a letter from
the Regulatory Body as they are deemed to be pre-registered.
