TEMPORARY WORKER APPLICATIONS

QUÉBEC ENGLISH LANGUAGE ASSISTANT PROGRAM

THE APPLICATION

In order for us to process your work permit application you will be required to submit the following documents.  The temporary worker application forms are available for viewing or download on the FORMS page.

1.  Application form - fully completed application form (IMM1295B),  Please include a day time telephone and fax number if possible, in case we need to contact you. A separate application form is required if your spouse wishes to apply for an open work permit. Please refer to the Spousal Work Permit page.

2.  Processing Fee - This class of work permit is exempt of any fee.

3.  Passport - a copy of the identity page of your passport.

4.  Photographs - two passport sized photographs. Please write your name on the back.

5.  Déclaration d'affectation - Copy of the "Déclaration d'affectation" issued by the Ministère de l'Éducation du Québec confirming your placement details in Québec.

You are required to have a medical examination before a work permit can be issued because you will be employed in an occupation in which protection of public health is essential. When we receive your application, we will issue your medical examination forms and instructions.
